(Ref LB2369 .G53 1995) The numbers in < > refer to the appropriate chapters in the han dbook. For example, for more information about about citing a source by two authors, see chapter 4.6.4.

Take the title from the title page, not the cover.
The author's name should be written Last Name, First Name.
