You must have all of the following records and documents to be enrolled. No
exceptions.
*Parent or legal guardian (as appointed by the courts) must accompany the
student. Legal documents must be provided in situations involving divorce
and/or legal guardianship. Parents may be asked to show their drivers
license.
*Proof of address of parent or court appointed guardian.
- If living in an apartment, a letter from apartment manager on apartment
letterhead stationery (A lease is NOT sufficient). This letter should stated
who the student lives with and in what apartment.
- The letter must be signed
by the apartment manager and contain a phone number where we may call them.
*Health Records
- TB skin test with results within the last 12 months!! Allow three days if
you have not had this test recently
- Diphtheria-Tetanus (DT booster in the last
ten years)
- Polio booster since the age of four.
- Measles, mumps, rubella (2 doses)
- Suggested to have begun hepatitis series of
shots.
*School records from your last school
- Report card from the previous semester.
- Unofficial transcript- must provide a translation if in another language
and include a grading scale.
- Withdrawal form from all Texas Schools if registering during the school
year.
*Birth certificate
*Social Security Card
*If you have an approved Transfer from H.I.S.D., you must bring a copy.
During peak registration times, registration will be appointment only. Allow
a minimum of two hours. It is a slow process to check all of your paper work.
